Team Registration

Teams must consist of 5 to 8 players. To register a team, find the league below that fits the players' grades or ages and download, print and complete the team sign up form. Cost per player is $20. Checks can be made out to: National MS Society and in the memo written: Dunk It. Cash will be accepted but checks are preferred. A waiver must be completed for each player. Players below the age of 18 must complete the minor’s waiver, while those over the age of 18 must complete the individual waiver (links are at bottom of page).

Forms can be mailed postmarked no later than March 12, 2009 to:

**LEAGUES ARE SUBJECT TO CHANGE CLOSER TO THE EVENT DATE DEPENDING ON SUPPORT! At least three teams must be signed up for each league to make each its own tournament. If anything is to be changed the team captains will be contacted with changes. Length of games will be determined depending on the number of teams registered.

ABA

The ABA is for players from grades 1 to 3, and is co-ed.
